Are you passionate about helping people regain their independence and confidence at home?
We’re looking for a dedicated Reablement Support Worker to join our responsive, multi-disciplinary Reablement Team on a temporary basis.
You’ll provide hands-on, person-centred care and therapeutic support to individuals following illness, injury, or hospital discharge — helping them rebuild their independence and continue living safely in their own homes.
Your Key Responsibilities:
Deliver personal, practical, and rehabilitative care in line with individualised care plans.
Carry out therapeutic activities and skills practice to improve mobility, daily living, transfers, and confidence.
Support clients to manage their home safely, maintain routines, and re-engage with social contacts and activities.
Accurately record and report changes in individual health, wellbeing, or circumstances to senior staff.
Communicate effectively — verbally and in writing — to ensure safe and coordinated care delivery.
Work as part of a 7-day service, ensuring clients receive timely support when they need it most.What We’re Looking For:
Valid UK driving licence and daily use of your own vehicle (essential for travelling across the Borough).
Experience working in a care, community, or reablement setting (desirable).
Ability to work independently and as part of a team, using initiative to report and act on concerns.
Good communication skills — both verbal and written — with confidence using IT and electronic monitoring systems.
Experience working with professionals, families, and other agencies is a plus.What We Offer:
