I am currently representing Slough Council, who are offering an initial temporary contract for a period of 3-6 months with a view to be extended or offered a permanent position for the right candidate at a rate which is negotiable dependent upon experience
We are looking for an Older Peoples Commissioner this role will be: SL1 2EJ Hybrid
The right candidate will:
This role is key to designing and implementing services for older people through high quality commissioning, including residential and extra care services.
The role is also responsible for leading on the Older People strategy and delivering the action plan through the established steering group as well as specific task and finish groups.
Work with colleagues across the Commissioning team to deliver and lead on complex commissioning and transformation projects within Adult Social Care, with a focus on services supporting older people and carers.
Recommissioning of the Extra Care (Care and Support component) and Homecare services including
* Homefirst – up to two weeks from hospital
* Reablement – up to six weeks – single hourly rate for each element of the process
* Review of pathway 2 D2A beds
* Long term homecare
Implement commissioning activities end to end in line with processes agreed with Head of Commissioning against strategic health and social care outcomes and the Commissioning forward plan.
Oversee activities to design and implement services for older people through high quality commissioning, including Homecare and extra care services.
We require the following:
* Experience of financial / budgetary management.
* Experience of working successfully in wider internal and external partnerships and collaborative ways of working to support the development or implementation of new and flexible ways of working.
* Significant practical experience acquired through a combination of training and working in a relevant area; managing across a technical or professional area, demonstrating self development and an enthusiasm for continuous learning.
