Job Summary:
We are looking for a dedicated and compassionate Adults Social Worker to work with our team to assess, plan, and deliver support services to adults who are experiencing difficulties in their daily lives. The successful candidate will have a strong understanding of adult social care and the ability to work effectively with a range of stakeholders, including service users, carers, and healthcare professionals.
Responsibilities:
Assess the needs of adults and develop individualized support plans
Work with service users to identify and achieve their goals and outcomes
Provide guidance and support to carers and families
Collaborate with healthcare professionals to ensure seamless transitions and coordination of care
Monitor and evaluate the effectiveness of support services
