I am currently representing Slough Council, who are offering an initial temporary contract for a period of 3-6 months with a view to be extended or offered a permanent position for the right candidate at a rate which is negotiable dependent upon experience;
We are looking for a Complex Needs Navigator this role will be: SL1 2EJ
The right candidate will:
The Navigator role will work with a small group of identified ex/rough sleepers within Slough, who require a multi-disciplinary approach to achieve sustainable outcomes. You will need to build trust, be able to provide a person centred support approach and advocacy to enable the clients to navigate pathways into services needed, to enable them to progress towards a life away from the streets and end repeat homelessness.
* Develop and deliver a service that is outcomes based and exceptional in a complex environment, by being creative, professional, holistic and getting results
* Work with colleagues to improve client’s life skills, to ensure they are able to sustain accommodation, improve health and take up work or volunteering.
* To facilitate joint working and pathways (including partners, peers and other agencies) to deliver focused support plans to maximize opportunities and outcomes for clients
* Engage with clients to motivate them to change away from a ‘street lifestyle’, utilizing different motivational techniques, reflective practice and an empathic attitude
* Identify the complex needs of often highly vulnerable and high-risk clients, which will include, substance misuse, alcohol dependence, mental health issues, offending and violent behaviour, anti-social behaviour, individuals involved in the sex trade and making appropriate and timely referrals.
* Produce high quality casework management forms/reports and maintain accurate record keeping
We require the following:
* Experience of working within – Outreach services, Housing Advice, Homelessness, Tenancy sustainment, Mental Health and/or Drug/alcohol service. Including case co-ordination and assessment, support planning and outreach.
* Experience of person-centred support planning, case management and accurate information recording
* Experience of working with ex/rough sleepers
